What is a Medical Record?


A Medical Record provides documentary evidence of the course of care received at HRMC. The Medical Record is property of the hospital. Medical Records are sent to the ordering medical department upon completion of services received. Medical Records must be completed by all physicians involved in a patient's care prior to being copied and released to the patient.

The role of the Health Information Management (HIM) Department is to maintain medical records in a standardized and professional manner in order to protect patient confidentiality. Transcription, diagnosis coding, and release of information are other major duties performed in the HIM Department. Information is also gathered and sent to state agencies concerning vital statistics.

How do I get a copy of a medical record?


Personal Medical Records are released in accordance with state and federal laws. There is a fee of 65 cents per page and 50 cents every page after 30 pages. The fee is waived if records have been requested by your physician. To receive copies of you or your children's records, you must first sign a release of information granting permission to release the records. You have the right to request records for your dependent children if they have not yet reached the age of 18. Once 18 years of age, a person must sign for their own records. Certain medical records are federally protected and cannot be released to the parent without the permission of the child if that child has reached the age of 14 in the case of reproduction or drug/alcohol abuse, and the age of 13 in the case of mental health. Certified birth certificates are NOT available from Hampton Regional's HIM Department. These records must be ordered from the Department of Vital Statistics of the state in which the birth was registered.
