Coastal Plains Primary Care at Hampton Regional Medical Center has received $20,000 as part of the 2022 Patient Centered Medical Home Recognition Excellence Award.

Varnville, SC - June 2, 2022 - Coastal Plains Primary Care at Hampton Regional Medical Center has received $20,000 as part of the 2022 Patient Centered Medical Home Recognition Excellence Award.
The South Carolina Office of Rural Health has partnered with the South Carolina Department of Health and Human Services to provide additional funding for practices who have been recognized as a PCMH. Providers must meet certain criteria and go through a stringent selection process to qualify.
CPPC Clinical Coordinator Tonya Faircloth is thankful for the funding, "This funding allows us to improve our practice with updated technology and equipment to keep us on the cutting edge of healthcare," she said.
According to Lindsay Williams from the South Carolina Office of Rural Health, DHHS awards practices who exemplify excellence in 20 criteria from the PCMH Standards that are felt to be the most vital in being a medical home. SCORH and SCMA both award 10 practices each through an application process and then provide facilitation to those selected practices to ensure proper processes and documentation is in place. They are then evaluated by the opposite organization to ensure they meet the 20 required criteria to receive the full funding.
"Coastal Plains has done a great job putting together a well-organized application, and is eager to serve the needs of their community. We are pleased to help further their mission to provide excellence in healthcare to rural South Carolina," Williams said.
